What is Hepcinat LP®?

Hepcinat LP is sofosbuvir 400mg and ledipasvir 90mg, exclusively manufactured by Natco Pharma. The global leader in manufacturing hepatitis C treatments around the world.

Hepcinat LP is a relatively new medication for the treatment of hepatitis C and it was released to the public on December 15th 2014.

This is a blockbuster drug that many around the world were waiting for to treat hep C genotypes 1, 4 and sometimes 3.  Its a one a day pill that cures hepatitis C.

Hep C treatment cost for Hepcinat LP is considerably lower than Harvoni, although molecularly identical, the costs of these treatments vary significantly.   It is often referred to as the “authorized generic Harvoni”.

US FDA Approved for Import

It was approved in US, UK and Europe in November 2014 for treatment of adults with genotype 1 or 4 chronic hepatitis C, and for some people with genotype 3.

Hepcinat LP is approved by the US FDA for the treatment of hepatitis C and is manufactured by Natco Pharma. The license to manufacture this hepatitis C treatment was provided by Gilead Sciences an American company.

New and Improved Hepatitis C Treatment

Treating hepatitis C has always been difficult due to the prevailing treatment options with peg interferon and ribavirin. Many hep C patients could not tolerate the treatment, which had a high failure rate.

Today, Hepcinat LP is truly a interferon free treatment option.

Getting treated for hepatitis C reduces your chance of liver cancer and the need for a liver transplant.

How does it work?

The two active pharmaceutical ingredients, sofosbuvir and ledipasvir, combat hepatitis C virus at RNA (similar to DNA) level. RNA is ribonucleic acid and DNA is deoxyribonucleic acid.

Hang in there, just a little bit more scientific jargon to explain how this hep C treatment really works.

Its All About the Protein!

Hepatitis C virus requires Protein A and Protein B to reproduce and replicate.  Sofosbuvir inhibits the virus to make the Protein A, ledipasvir interfers and blocks Protein B from being manufactured inside cell walls of the virus.  Together, sofosbuvir and ledipasvir work in unison to halt the replication and reproduction of the hepatitis C virus.

A familiar example at home is the art of bread making. When we make bread, we need two important ingredients, yeast and sugar.  Sugar is required by the yeast to make the dough rise, with the release of carbond dioxide and ethyl alcohol.

Without getting too compicated about the art of baking bread, sugar and yeast are important to make a good loaf of bread.  Just as sofosbuvir and ledipasvir are important to destroy certain RNA profiles of the hepatitis C virus.

Those certain RNA profiles are referred to as hepatitis C virus genotypes.

You got it.

Hepcinat LP targets genotypes of 1, 4 and sometimes 3. You will see as you read along, that Hepcinat LP can be used to treat more genotypes with combinations of ribavirin.

Who can use this hep C treatment?

Any adult with chronic hepatitis C (minimum of 6 months infection), that have genotypes 1, 4 and some with genotype 3.

Europeans, majority, have hep C virus genotype 1.

Treatment-naive patients, those that have not been treated for hep C virus, can take Hepcinat LP.  Those that have been treated with interferon based therapy and not cured, can also take this hep C treatment.  These individuals are called treatment experienced.

Good News for HIV-HCV Co-infection Patients

Another group of individuals that can benefit from Hepcinat LP are those with HIV and HCV co-infection. The results of the treatment in this group is positive and encouraging because they cleared the hep C virus.

This drug works with all current HIV medications, there are no contraindications, works just fine.

Final group of individuals that can benefit from taking this medication are those with compensated and decompensated cirrhosis. Also, individuals with a liver transplant and those waiting for a liver transplant.

All hepatitis C virus positive individuals should be under the care of a doctor when taking Hepcinat LP.

How is it taken?

You take the once a day pill with or without food. It depends on your preference and what you are comfortable with.  Its also important to consider the length of treatment with Hepcinat LP and also your treatment history.

Take a look at the boxes below as a guide for treatment.

Genotype 1, 4, 5, 6

Hepcinat LP

-8 weeks (no cirrhosis, no previous treatment)

-12 weeks (no cirrhosis, previously treated)

-24 weeks (no cirrhosis, previous treatment, lack of future options)

-24 weeks (cirrhosis or post-transplant with cirrhosis)

Genotype 1,4,5,6

Hepcinat LP with Ribavirin

-12 weeks (no cirrhosis, previous treatment or post- transplant)

-12 weeks (cirrhosis, including decompensated cirrhosis)

Genotype 3

Hepcinat LP with Ribavirin

-24 weeks (cirrhosis and/or previous treatment)

This hepatitis C treatment is not approved for people with HCV genotypes 2, 5 or 6. People with genotype 2 should use sofosbuvir (Sovaldi) plus ribavirin.

Recommended uses for people with HIV and HCV co-infection are the same as for HIV-negative people.

How effective is it?

It really depends on a variety of factor; treatment history, liver damage and HCV genotype.

If you have mild or moderate level of fibrosis, Hepcinat LP can be very effective form of treatment compared to advanced liver disease.

Treatment experienced individuals will not respond as well as those that never had treatment for hepatitis C.

For treatment experienced individuals, a longer treatment is recommended or adding ribavirin to the treatment to prevent relapse.

What are the drug interactions?

Do not take this drug with Crestor (rosuvastatin) or St. John’s wort containing herbal products.

HIV drug tenofovir levels are increased when taking Hepcinat LP. Kidney function should be monitored regularly.

Case Studies with Hepcinat LP Treatment

Case Study #1

HCV genotype 1 – Hepciant LP cured 94% to 100%, both previously treated and untreated patients.

This data is based on hepatitis C virus load test after 12 weeks of treatment, also referred to as SVR12 or sustained virological response 12 weeks.

Case Study #2

HCV genotype 1 or 4 – Hepcinat LP and ribavirin taken for 12 or 24 weeks cured 87% to 89%, both previously treated and untreated patients with deompensated cirrhosis.

The same treatment was given to those with liver transplants and had a success rate of 95%.

Case Study #3

HCV genotype 3 – Hepcinat LP and ribavirin for 12 weeks, 100% cure rate.

*without ribavirin cure rate was only 64%

Case Study #4

HIV-HCV Co-infection Genotype 1 – Hepcinat LP for 12 weeks, 98% cure rate, previously untreated and no cirrhosis.
